Certified birth certificates
When a child is born at The Medical Center of Plano, we submit the birth certificate information to the Bureau of Vital Statistics. Certified birth certificates can only be obtained through the bureau, which is part of the Texas Department of State Health Services. The hospital does not have a certified copy on file.
Obtaining a certificate
For parents or patients over the age of 18: To obtain a birth certificate, submit an application to the Bureau of Vital Statistics. You can do this one of three ways:
- Complete the application on the Texas Department of State Health Services Web site, and submit it to the Collin County Clerk's Office, 900 E. Park, Suite 140C, Plano, TX 75074. Phone: (972) 881-3025
- Complete the application on the Texas Department of State Health Services Web site, and submit it to the Bureau of Vital Statistics, DSHS, P.O. Box 12040, Austin, TX 78711. Phone: (888) 963-7111
- Visit one of the remote sites listed on the Texas Department of State Health Services Web site.
Copies of medical records
To obtain a copy of your medical records, complete the Release of Information (ROI) Form, and provide a copy of your driver's license. Submit the information one of two ways:
- Fax to (469) 484-2006.
- Mail to The Medical Center of Plano, ROI Department, 508 Wrangler, Suite 200, Coppell, TX 75019.
The ROI office is staffed Monday through Friday from 8:30 a.m. to 4 p.m. and can be reached at (972) 906-1759. Calls left after business hours will be returned the next business day.
